Mental Health Practitioner/ Care Co-Ordinator Band 6 Job Description

• Early Intervention Service 
• To provide an intensive outreach service to young people aged between 14 and 35 and their families who have experienced a first episode of psychotic illness and to work with them over 3 years
• North West London
• 6 months (possibility of going perm)
• Up to £24 per hour depending on experience

Requirements

• Must have experience CCO experience
• Must have experience working with 14-35 year olds
• Must have experience working with psychosis
• DBS or Police Check
• Registered with relevant goverment body 
• RMN, SW, OT, Pscyhologist with relevant professional body Registration (NMC, HCPC, SWE)
